Medicaid Service Coordination

Block Institute is proud to offer Medicaid Service Coordinators (MSC), to OPWDD Eligible individuals who are in need of ongoing and comprehensive case management services. This Medicaid funded service provides assistance to individuals with Intellectual Disabilities with the goal of helping the individual and their families obtain services that will help the person to gain their independence and support their life within their community. Through MSC service, individuals can develop, implement, and maintain their life plan in order to achieve a higher and more positive quality of life.
When the individual and or their family decide to participate in the MSC program, a Service Coordinator will be offered to them. The role of MSC is to keep the individual and their family’s interests first as well as serve as the individual’s liaison in promoting a person centered agenda.
Individuals and families who receive MSC are entitled to a wide variety of services such as respite services, clinical services, day habilitation, residential and supported employment.
